15  Observed the following improper non-food contact surfaces: Wheels to the shelves in the walk-in cooler are broken. Provide for all food contact surfaces to be smooth, easily cleanable, non-absorbent, corrosive resistant, and light colored. Next Inspection
17  Provide for the test strips for the quat based sanitizer to be available at all times for proper testing. Next Inspection
22  Observed the following food contact surfaces to be unclean: Fryer scoop holder above the fryers was filthy, 2) Inside of the milk shake cabinet down below was dirty. Clean and maintain Next Inspection
23  Observed the following contact surfaces to be unclean: 1) Handle to one of the bun drawers, 2) Handles to the milk shake glass freezer as well as all the handles throughout the kitchen, 3) Outlet by the milk shake area is covered in peanut butter. Clean and maintain Next Inspection
36  Observed the following floor areas to be unclean:
1) Under the milk shake counter,
2) In front of the dirty dish window,
3)Under the burger topping counter in cook line,
4) In the walk-in cooler under the racks the grooves in the tiles need to be scrubbed,
5)Under the dish washer,
6) In the mop room around the drain. Provide for all flooring areas to be in a clean condition. Observed following floor areas to be in poor repair: Tile that was loose in the back area by the back garbage door needs to be secured better with some more caulking. Provide for all flooring areas to be smooth, easily cleanable, non-absorbent, durable surfaces, and be maintained in good repair

37  Obseved the following wall/ceilings, and attached equipment to be unclean: Walls throughout the facility( cook line, dish washing window, prep corner) have some kind of food splatter. Clean Next Inspection
